Reduced Rate Program Forms
The City of Louisville provides a program for eligible participants, which will reduce fees by 50% for activities and/or passes sponsored by the Parks and Recreation Department. This reduced rate program is available to all Louisville families who meet one of the following guidelines:
Recipients of Aid to Dependent Children
W.I.C. Recipients
Medicaid Recipients
Section 8 or Public Housing
Reduced Rate Program Application
