2012 Summer Day Camp (K-5th Graders for 2011-2012 School Year)
Summer "Play" Day Camp, based at the Louisville Recreation Center, has the perfect mix of structured and child directed activities! Arts and Crafts fosters creativity, free thinking and self expression. Sports and games provide opportunities for teamwork, new experiences and physical activity. Free choice time allows for child-centered play for healthy brain development, imagination, physical strength and cognitive skills. Plus weekly field trips (Wednesdays,) swimming at the Rec (Tuesdays) and Memory Square (Fridays) and the best counselors! No wonder campers return year after year! Take a look at calendars from previous years. Themes and activities are designed with the elementary aged child in mind. We ask parents to use their best judgment in considering enrolling a younger child, unaccustomed to a large setting with 60 children of various ages; as well as older children who may not be interested in being in a camp with various ages. Be sure to read the Summer Day Camp Handbook for specific policies and procedures. Register early!

Campers are broken into 2 groups by age for small group activities and they rotate activities

Priority Online Registration (Louisville Residents Only) February 14-17, 2012.
Regular Resident Registration Begins March 1, 2012.
Regular Non Resident Registion Begins March 8, 2012.

Please have immunizations recorded on this form or a form that says "Approved by CDPHE." If your doctor submits immunizations on a non approved form, please transcribe onto this form and attach the doctor's copy as well.

Remainder of camp tuition is due May 17, 2012 - You may pay your remainder online at www.louisvillerecreation.com anytime between Monday, May 14, 2012 and Thursday, May 17, 2012 or at the front desk during the open house.

Medications (Prescription or Over the Counter)
  • Use the following forms ONLY if your child will need medication DURING camp hours. It is best to arrange to take as many medications at home as possible as we do not have a nurse on staff.
  • One form per medication.
  • No medications in camp without correct forms signed by both parent and doctor and medication in the original labeled container (Epi pens may be in their plastic tube with a pharmacy label on the outside- this is our preference as it makes them much easier to transport- thank you!).
  • Please make clear to your physician that they are filling out the forms for a lay person (camp staff member), NOT a nurse. They must be very clear and black and white with their directions as camp staff are not able to make any medical decisions.
  • Please read each explanation to make sure you fill out the correct form.

Allergy Action Plan is used for severe allergies and Epi Pens
Asthma Care Plan is used for any inhaled medications to include albuterol inhalers
Medication Administration is used for all general medications
Exemption for Medication is used if your child has a noted medical condition, but you choose not to send any medications to camp

2012 Camp Calendars
Your Head Camp Counselors will begin planning in the Spring and detailed calendars will be available in May.




Please read before registering for youth activities
The Louisville Recreation Center provides a variety of children's enrichment programs designed to stimulate your child's social, physical, and cognitive skills. Programs are designed with specific age groups in mind to provide the best in program quality. Please make certain your child is in the specified age required for a program by the first day of class. Children will be canceled from a class roster if they do not meet age requirements.

Summer Camp Cancellation/Transfer Policy: Once a child is enrolled, there will be a surcharge for EVERY session cancellation or transfer. Cancellations must be requested in writing by 9am the Monday BEFORE the session begins, otherwise you will forfeit the ENTIRE session fee. Cancellation/Transfer fee: M-F fee $25, MWF or Tu/Th fee $15.00. Refunds $20.00 and under will remain as a household credit. Refunds over $20.00 can be processed as a credit refund or check.
